  1. Log in to HealthLink through your practice management system (Socrates, HealthOne, Helix, etc.)

  2. Select Private Hospital

  3. Choose St John of God University Hospital – Inpatient Services

  4. Submit the referral

  3. Choose St John of God University Hospital – Outpatient Services.

Involuntary Referrals

Care and Treatment for Involuntary Service Users: At St John of God University Hospital, we ensure compassionate and comprehensive mental health care. Involuntary admissions are conducted strictly in accordance with the Mental Health Act to safeguard the individual and those around them.

Important: All referrals must be submitted via our encrypted email address: [email protected]. We no longer accept referrals via fax. Please ensure that all documentation is scanned and attached to a single secure email.

Referral Instructions

  • If the person being referred is under the care, or was recently under the care, of another Consultant Psychiatrist or Psychiatric Service, the referral must be made by that Consultant or Service.

  • Download and complete our Referral Form by [insert link or reference to website/download location].

  • Ensure the appropriate Application Form (Form 1, 2, 3 or 4 – Applicant) and Recommendation Form (Form 5 – GP) are filled in accurately and within 24 hours of each other.

  • Email the completed forms and referral documentation to [email protected].

Additional Notes

  • These forms must be inspected by the Hospital’s authorised staff before the patient can be admitted. Inaccurate or incomplete forms may result in delays.

  • The original Mental Health Act paperwork (Forms 1–5) must accompany the patient to the Hospital with either a family member or the Assisted Admission Team. These are statutory documents required for legal detention.

  • To arrange the assistance of an Assisted Admission Team, please contact the Admissions Office at 01 277 1450 (Monday to Friday, 8.30am–8.00pm) or 01 277 1400 (outside these hours and at weekends).

Mental Health Commission Forms 1–5 can be downloaded as follows:

Form 1: Application (to a registered Medical Practitioner) by Spouse or Civil Partner or Relative
Form 2: Application (to a Registered Medical Practioner) by an Authorised Officer
Form 3: Application (to a Registered Medical Practitioner) by a member of An Garda Síochána
Form 4: Application (to a Registered Medical Practitioner) by any other person
Form 5: Recommendation (by a Registered Medical Practitioner)

* Assisted Admission Team

This is a specialised statutory service designated by the Irish Mental Health Act 2001/09. The service provides a safe and discreet client and family-centred service throughout the country, which assists the person with mental health and/or learning disability issues and who is unwilling to go to hospital voluntarily for assessment.

  • Proper Documentation: Accurate completion of all necessary documentation.

  • Qualified Assessment: Thorough assessments by qualified mental health professionals.
